Permission Slip to Hunt

Does anyone know where I can get or print off some generic but still legal slip I can sign and give to a land owner saying I wont hold them responsible if I get hurt while hunting on their land? I know some states require it but Missouri doesn't, and I want to Show the landowner I take the prevledge serious.

Look on Ohio's dnr site I know they have a permission to hunt form but I don't think it has a liability exclusion, you may just write something up your self.
